From best-case scenario to worst... One possibility is that your air pressure is just too low. Have you tried increasing the air pressure above what you think it should be and seeing what happens? Sag is a useful indicator but not a perfect standard for rear shock pressure. If adding more air helped, it would be the simplest and cheapest fix for the problem. Or perhaps you have a pressure leak. If the shock hasn't been serviced recently, worn out air sleeve seals could be causing the shock to lose pressure after you set your sag. One way to check...
