Thank you for the upcoming nightmares.

I will say bolts loosening on ebikes is a pain, I need to toque check my rear end after every ride, between the pivot bolts and the rear axle you need to over torque everything to make through a big ride.

That is a RaceFace Vault hub - according to the owner "new" - it sure looked new. Now it is new...and bustified :-)

loose brake bolts with a 60lb. bike, what could go wrong?

I'm going to say the bolts were loose and once 4 of them were out the 2 remaining tabs had o chance.

Going by the way those CPS washers have been installed (under the mount instead of the calliper) I don't have a lot of faith that anything was mounted correctly

I am surprised you didn't end up with your teeth rubbing on the front tire. Luckily the rear wheel didn't lock up on you. This is just not paying attention to normal maintenance. The bolts had to have been loose as shit for a while to have this happen. Maybe that was the rattling sound you were hearing for the past 3 rides. Has nothing to do with E-bike hubs. Has to do with the e-bike rider.
