The material of the insert is far tougher than an inner tube, I have pinch punctured actual tyres without puncturing the OPTIS. IF you could somehow puncture it, it can be patched with standard TPU patch kits and some sealants like OKO will work too. But I would emphasise that puncturing it will take a huge impact. The reason for selling bladders separate to the valve is for people changing wheel size.

A skinny tube is going to inflate to fill the entire tyre at a single pressure. The OPTIS makes your tyres progressive so you can run low pressure for high grip AND have bottom out protection, bead-lock and tyre support.

Main pawl seal does indeed look like zero drag, but it also looks like zero seal. Rebuilding a friend's hub this is always his biggest problem. Be nice if they offered a proper seal (at the expense of some extra drag) for those of us who ride through British winters.
